1) ID books of both parties.
2) Declaration for the Purpose of Marriage (declared with me)
3) Consent to the marriage of a minor (if underage)
4) Final Decree of Divorce (if applicable)
5) Passports (if foreigners)
6) Declaration pertaining to customary unions (if prospective husband is a black person)
7) Declaration pertaining to community of property (if black person)
8) Death Certificate (if widow or widower)
All the forms is available and can be declared with me.(Consent to the Marriage of a Minor and
Declarations.)
If a black male desires to enter into a legal wedding in addition to a traditional wedding and he is in
more than one traditional marriage relationship,a certification is to be made at a Magistrate with regard
to the relationship that subsist between him and any other woman than the one he intends to legally
marry. This certificate must then be handed over to me.
